Procedures
- The applicant shall submit the old card to the Reception Desk officer.
- The applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant pays the prescribed fees at the cashier’s counter.
- The Applications Intake officer enters data.
- The auditor checks the validity of the card.
- The application is sent electronically to printing department.
- The new card is printed and handed over to the Reception Desk officer.
- The Reception Desk officer then gives the new health card to the applicant.
Required Documents
- The old card.
- One (1) personal photograph.
